Hi there!
Yeah since the smoke overlay effect with that particular type of item is still technically one item to draw, I wouldn't count it as two separate additional costs. Your total would be:
Base price: 200
Additional clothes: 5 x 50 = 250
Total: 200 + 250 = 450 notes

I'm actually wondering if it's possible to have two images, one with just the Runestone Pendant and one with all the items below? I'm not sure how much this would add to the original cost.

With just the default, I think this would be 245 (100 plus the 40 each for 3 unlocked items and 25 for the petal whirl), but please let me know if the double file thing adds anything.

Since with the way I do clothing additions to the pixels involves different layers on the main art file I have made, it wouldn't cost anything additional to make an alternate version of the pixel. It would just involved hiding the layers the other three pieces of decor are on and saving two different files, so I wouldn't charge you extra for that. Your total is correct as well! It would be:
Base price: 100
Locked decor: Not-Holly Earpiece - Periwinkle Tail Corsage, Rune Pendant = 40 x 3 = 120
Unlocked: Petal Whirl = 25
Total: 100 + 120 + 25 = 245 notes
